Booking, cancelling or changing a dental appointment is easy. Just call or drop by and let the front desk know. We always work with you to provide treatment options that meet your personal needs and budget.
You will be asked to fill out a pre-appointment form before we treat you. Please arrive early because it will take some time to complete. Alternately, you can fill out the form before you come. It can be downloaded for you to print and bring with you. You may also use the following online pre-appointment form.
We require at least 24 hours notice of an appointment cancellation. Failure to provide adequate notice may result in a missed appointment fee.
As a courtesy for patients with dental insurance, we can submit your claim directly to your insurance carrier via email, as long as all pertinent information is provided.
Existing patients with dental emergencies may contact us after hours. The emergency contact number is recorded on our answering machine. You may call the office at the time of your emergency and find out what it is if you are experiencing significant pain, swelling, or injury.
